Prime Path Logistics & Services LLC
These Terms of Service apply to all services provided by Prime Path Logistics & Services LLC, including Medical Courier, Specimen Collection, Medical Supplies, Logistics & Delivery, Equipment Delivery, and Mobile Notary Services.
By scheduling or requesting service, the client agrees to the terms outlined below.
SERVICE AGREEMENT
All services are provided based on availability and confirmed scheduling. Prime Path Logistics & Services LLC reserves the right to accept, reschedule, or decline any service request.
Final pricing is confirmed via invoice prior to dispatch.
PAYMENT TERMS
- Payment is required prior to dispatch for all services unless a written commercial agreement is in place.
- All services are invoiced through Square via text or email.
- A 3.5% processing fee applies to all credit and debit card transactions.
- Applicable North Carolina sales tax may be applied where required by law.
TRAVEL POLICY
Mobile services include travel fees calculated based on distance and service type. Travel is non-refundable once dispatch or travel has begun.
CANCELLATION POLICY
Standard Cancellations
- Cancellations must be made at least 2 hours before scheduled service time
- No fee applies if cancelled within this timeframe
Late Cancellations
- Cancellations made less than 2 hours before service may incur a $25–$75 fee
Same-Day / On-Demand Services
- Once dispatched or en route, cancellations are non-refundable
STAT / Emergency Services
- STAT and emergency requests are non-refundable once payment is received or dispatch begins
NO-SHOW POLICY
- If the client is unavailable at the scheduled location or does not respond within a reasonable time frame:
- A no-show fee of $50–$100 may be applied
- Travel fees may still apply
- The service will be considered completed or cancelled at provider discretion
WAIT TIME POLICY
- A 30-minute grace period is provided for all appointments
- After 30 minutes, a $25 wait fee per additional 15 minutes may be applied
- Extended delays may result in cancellation with applicable fees
SERVICE LIMITATIONS
Prime Path Logistics & Services LLC reserves the right to refuse or terminate service if:
- Unsafe conditions are present
- Items exceed vehicle or handling capacity
- Proper documentation is not provided (medical/legal services)
- Client becomes unresponsive or uncooperative
COMMERCIAL ACCOUNTS
Approved business clients may qualify for:
- Net 15 / Net 30 billing terms
- Volume-based pricing
- Contracted service agreements
All commercial agreements must be documented in writing and approved prior to activation.
LIABILITY POLICY
Prime Path Logistics & Services LLC is not responsible for:
- Improperly packaged items
- Delays caused by traffic, weather, or third parties
- Pre-existing damage to items
- Incorrect or incomplete client instructions
All items are handled with reasonable care during transport and service.
SERVICE AREA
Services are provided within North Carolina and surrounding regions based on availability and travel feasibility.
AGREEMENT ACKNOWLEDGMENT
By requesting service, the client acknowledges they have read, understood, and agreed to these Terms of Service and Cancellation Policy.
